Home Health Occupational Therapist OT Full Time

Job Summary
Assess, teach, and deliver occupational therapy interventions directed by the physician within home health plans of care. Perform initial and ongoing assessments of mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations defining needs, progress, and expected outcomes, ensuring documentation complies with policy and minimizes payer denials. Collaborate with interdisciplinary teams, physicians, and caregivers to optimize patient well-being, modify treatment plans, and coordinate discharge care including home exercise education. Supervise other personnel as applicable and complete OASIS assessments where permitted. Monday through Friday full-time visits across Clarke, Frederick, and Warren counties.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
- Valid Drivers License
