Job Summary
Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations, modify treatment plans, and manage discharge care including educating patients and caretakers on home exercise plans. Collaborate with interdisciplinary team members, physicians, and nurses to optimize patient well-being, maintain OASIS documentation, and supervise other personnel as applicable.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
