Home Health Occupational Therapist OT Full Time 15K Bonus

Assess, teach, and deliver therapy interventions directed by the physician within the home health plan of care. Provide written evaluations defining needs, goals, and outcomes while maintaining plans through timely documentation and modifying treatment as needed. Collaborate with interdisciplinary team members, physicians, and caregivers to optimize patient well-being, coordinate services, and educate families on discharge home exercise plans. Ensure all documentation complies with policy and payer requirements to prevent denials, serving as a clinical resource consultant for staff. Requires current state license, OASIS documentation experience, and full-time status with a 15K sign-on bonus.
