Home Health Occupational Therapist OT Full Time 10K Sign On Bonus

Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations, modify treatment plans, and collaborate with interdisciplinary teams, physicians, and caregivers to optimize patient well-being and coordinate discharge planning. Maintain accurate documentation to ensure compliance with physician orders and payer requirements, supervise other personnel as applicable, and complete OASIS assessments where permitted. Full-time visits Monday through Friday in Polk County.
