Home Health Occupational Therapist (OT)

Provide skilled occupational therapy in accordance with the physician's plan of care, accepting only patients referred by the patient care manager. Assist the physician in evaluating the patient's level of functioning through diagnostic procedures, setting goals, and revising the plan of care. Design orthotics as indicated by the patient's condition and treat patients using therapeutic activities to restore function and promote independence. Observe, record, and report patient reactions and condition changes to the physician and case manager while identifying barriers to desired outcomes. Visit rates range from $63.00 to $100.00 per visit with mileage reimbursement, and the position requires an active Michigan Occupational Therapy license and reliable transportation.
