Job Summary

Conduct one-on-one occupational therapy sessions with patients in their homes, assisted living facilities, or nursing homes to help them regain independence and return to daily activities. Assess functional abilities, design personalized treatment plans, and develop adaptive strategies for tasks ranging from dressing and bathing to cooking and managing routines. Provide hands-on training, education, and encouragement while collaborating with families and interdisciplinary care teams to support patient progress toward greater confidence and safety. This role requires daily travel within the Boone, Clinton, and surrounding Indiana service areas, with a Monday-to-Friday schedule and weekend on-call rotation. Required Qualifications

  • Graduate of an accredited occupational therapy program (ACOTE accredited)
  • Current state license as an Occupational Therapist
  • Minimum 1 year of experience as a licensed Occupational Therapist
  • Reliable transportation, valid driver's license, and valid automobile insurance coverage
  • Willingness to travel daily for home visits within service area
  • Monday to Friday, 8 a.m. to 5 p.m.
  • Weekend On-Call Rotation
  • Employment is contingent upon the successful completion of a background check

Desired Qualifications

  • Knowledge of Medicare home health documentation
