Home Health Occupational Therapist

Conduct screenings, testing, evaluation, diagnosis, and treatment of injuries, diseases, and disabilities using occupational therapy procedures and modalities. Consult, educate, and train patients, families, and caregivers while collaborating with care teams to deliver patient-centered care. Develop care plans with specific goals based on patient assessments and participate in interdisciplinary meetings and quality improvement processes. Manage all aspects of patient care and supervision, including overseeing assistants, aides, and students. Maintain thorough patient records and complete billing accurately. Promote continuing education opportunities and contribute to program development strategies. Full-time role with 40 hours per week, offering flexibility for holidays or weekends.
