Home Health Occupational Therapist Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Administer, evaluate, and interpret tests to determine functional goals, then deliver occupational therapy services while complying with prescribed physician orders. Provide written evaluations and plans of care defining needs, goals, progress, and expected outcomes. Maintain plans by documenting client performance, modifying treatments, and determining discharge care plans. Collaborate with interdisciplinary team members, physicians, nurses, and families to optimize patient well-being and coordinate services. Supervise other personnel as applicable and complete OASIS assessments where permitted. Ensure documentation adheres to policy to prevent technical or clinical denials.
