Job Summary
Occupational Therapist needed for home health in Sioux Falls and surrounding areas. Responsibilities include performing initial and ongoing assessments of mobility, coordination, strength and function; delivering OT services per physician orders; developing and maintaining a written plan of care with goals and progress; discharge planning and home exercise guidance; coordinating with the care team and family; maintaining timely documentation and OASIS where allowed; supervising OTA/HHA as applicable; and contributing to patient education and care coordination. Requires an OT degree from an accredited program and an active state license, plus CPR, a valid driver’s license, and proof of automobile insurance. Preference for experience in home health therapy and OASIS documentation.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
- Drivers License and Proof of Automobile Insurance
