Job Summary
Provide in-home occupational therapy evaluations to assess functional limitations, safety risks, and ADLs/IADLs; identify factors impacting independence; educate patients, families, and caregivers on therapy goals and home exercise programs; monitor progress and adjust treatment plans; collaborate with a multidisciplinary team; maintain accurate documentation; coach and support caregivers. Mileage reimbursement and flexible schedules offered; opportunities for full-time or part-time positions with company-paid life insurance and 401k; in-home service delivery with Mesa, AZ area focus.
Required Qualifications
- Active Occupational Therapy License (Arizona)
- Valid Driver's license
- Reliable transportation
