Job Summary
Conduct home visits to geriatric clients in senior living communities across Mansfield, Attleboro, Easton, and Norton. Perform thorough evaluations of patients' functional status, including muscle function, endurance, and ADL performance, while assessing home environments for hazards. Develop individualized intervention plans and educate patients or family members on rehabilitative care. Continually revise care plans and accurately document observations using a touch pad tablet. This role supports a multi-disciplinary team providing skilled nursing and rehabilitative care to help clients remain at home.
Required Qualifications
- A current Massachusetts Occupational Therapist license
- A minimum of one year of recent work experience as an Occupational Therapist

- Prior home care experience
