Job Summary
Provide therapy services under the supervision of a licensed Occupational Therapist in patients' homes, implementing treatment plans to improve strength, mobility and functional independence. Design/adapt equipment and living environments, fabricate devices to assist function, instruct patients and families on home programs and use of adaptive equipment, document observations and prepare progress reports, participate in care coordination and discharge planning, and coordinate plans with the interdisciplinary team. Requires OTA licensure, CPR certification, and a valid driver's license with reliable transportation; TB screening and driver-safety program compliance are noted as part of Humana CenterWell Home Health.
