Home Health Occupational Therapist Assistant

Provide therapy services planned and supervised by a qualified Occupational Therapist to help patients regain strength, mobility, and independence. Assist in implementing vocational programs, design adaptive equipment, and instruct patients and families on home care routines. Deliver therapeutic treatment and instruction to improve function while reporting observations to clinical supervisors and documenting progress in patient records. Coordinate discharge planning and community reintegration activities with the interdisciplinary team. Maintain all documentation as required by the company and facility.
