Job Summary
Provide therapy services planned and supervised by a qualified Occupational Therapist to help patients regain strength, mobility, and independence in their homes. Assist in implementing vocational programs, design adaptive equipment, and instruct families on home care routines. Report observations to clinical supervisors, document patient progress, and participate in care coordination and discharge planning. Maintain accurate records and submit documentation as required. This role supports CenterWell Home Health's mission to deliver personalized, comprehensive home care for patients managing chronic conditions or recovering from injury.
Required Qualifications
- Current and unrestricted OTA licensure
- Current CPR certification
- A valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (patient-facing role)
Desired Qualifications
- Minimum six months occupational therapist assistant experience
- Home Health experience
