Home Health Occupational Therapist Assistant

Provide therapy services planned and supervised by the qualified Occupational Therapist to help patients regain strength, mobility, and independence from the comfort of their homes. Assist in implementing vocational programs, design and adapt equipment or living environments, and fabricate devices to improve function and community participation. Deliver therapeutic treatment and instruction to patients and family members regarding home programs and adaptive equipment use. Report observations to the clinical supervisor, document clinical notes, and participate in care coordination and discharge planning activities. Maintain and submit documentation as required by the company and facility.
