Home Health Occupational Therapist Assistant

Deliver therapy services planned by the qualified Occupational Therapist to help patients regain strength, mobility, and independence in their homes. Assist in implementing vocational programs, design adaptive equipment, and instruct patients and families on home care and device usage. Provide therapeutic instruction to improve function, report observations to supervisors, document clinical notes, and participate in care coordination and discharge planning. Maintain patient records and submit required documentation while adhering to physician orders and company protocols.
