Job Summary
Provide therapy services planned and supervised by a qualified Occupational Therapist to help patients regain strength, mobility, and independence in their homes. Assist in implementing vocational programs, design and adapt equipment, and instruct patients on home care and adaptive devices. Report observations to supervisors, document clinical notes, and participate in care coordination and discharge planning with the interdisciplinary team. This role supports CenterWell Home Health's mission to deliver personalized, comprehensive home care for patients managing chronic conditions or recovering from injury.
Required Qualifications
- Current and unrestricted OTA licensure
- Current CPR certification
- A valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
Desired Qualifications
- Minimum six months occupational therapist assistant experience
- Home Health experience
