Home Health Occupational Therapist Assistant

Deliver therapy services planned and supervised by a qualified Occupational Therapist to help patients regain strength, mobility, and independence from the comfort of their homes. Assist in implementing vocational and education programs designed to restore task performance and promote self-sufficiency while designing or fabricating adaptive equipment to improve function. Provide therapeutic treatment and instruction to patients and family members regarding home programs and care of adaptive devices as directed by physician orders. Report observations to clinical supervisors, document findings in patient records, and prepare progress reports. Participate in care coordination, discharge planning, and community reintegration activities to support patient recovery and psychological adjustment.
