Home Health Occupational Therapist Assistant

Provide therapy services planned and supervised by the qualified Occupational Therapist to help patients regain strength, mobility, and independence. Assist in implementing vocational programs, design adaptive equipment, and instruct patients and families on home care and adaptive devices. Fabricate devices to improve function and participate in care coordination, discharge planning, and community reintegration activities. Report observations to supervisors, document clinical notes, and prepare progress reports in accordance with physician orders. Maintain documentation as required by the Company and facility.
