Job Summary
Administer occupational therapy evaluations, develop Plans of Care, and establish treatment goals for clients in their homes. Provide occupational therapy treatments, procedures, and diagnostic tests under physician direction while translating exercises into functional activities of daily living. Develop equipment modification plans, instruct families on home therapy programs, and prepare clinical progress notes. Supervise COTAs, HHAs, LPNs, and RNs as appropriate, participate in case conferences, and maintain confidentiality. Maintain a safe client environment and report any suspected abuse or neglect. Requires current Minnesota Occupational Therapist license, two years healthcare experience, and full-time or part-time flexible scheduling Monday through Friday.
Required Qualifications
- Currently licensed as an Occupational Therapist in the state of Minnesota
- The license must not have been revoked, suspended, and without limitations or restrictions
- Graduate of AOTA approved school of occupational therapy
- Currently certified by the AOTA to practice occupational therapy
- At least two (2) years experience in a health care setting
- Home care experience
- Access to a dependable vehicle or public transportation
- A valid driver's license
- Proof of car insurance when using a personal vehicle for company business
- Proof of negative mantoux or documentation of negative chest x-ray
- U.S. Citizenship or evidence of valid Alien Work Permit
- Discloses any conviction and criminal history records pertaining to any crime related to the provision of health services
- Pass initial and ongoing background studies and screenings including but not limited to those of the Minnesota Department of Health and the Federal Office of the Inspector General's List of Excluded Individuals and Entities
Desired Qualifications
- Certification with National Board of Occupational Therapy
- Current CPR certification
- Home care experience
