Job Summary
Occupational Therapist delivers home-based OT services, including evaluations and treatments, to clients in their homes within the designated coverage area. Responsibilities include developing and updating the Plan of Care, achieving functional goals, translating exercises into activities of daily living, instructing families, supervising therapy-support staff as appropriate, maintaining thorough documentation, and collaborating with physicians and the Area Clinical Manager. Requires travel to clients’ homes (1:1 care, 1–3 hours per client), adherence to confidentiality, safety, and professional standards, and active communication with clients, families, and team members. Eligible candidates must be licensed in Minnesota, graduate from an AOTA-approved program, have minimum healthcare experience (home care preferred), and possess strong communication and organizational skills; valid driver's license and vehicle access are required for travel.
Required Qualifications
- Licensed Occupational Therapist in Minnesota
- Graduate of AOTA approved school of occupational therapy
- Currently certified by the AOTA to practice occupational therapy
- At least two (2) years experience in a health care setting or equivalent (home care preferred)
- Strong interpersonal communication and teaching skills
- Strong organizational skills and ability to work independently
- Excellent written and oral communication skills
- Access to dependable vehicle or public transportation to travel to multiple company stops
- Valid driver’s license and proof of car insurance for personal vehicle use
- Proof of negative mantoux or chest x-ray as required
- Current CPR certification recommended
- U.S. Citizenship or evidence of valid Alien Work Permit
- Background checks and screenings as required
- Discloses any conviction and criminal history records pertaining to health services; will affect hire
Desired Qualifications

- Licensed Occupational Therapist in Minnesota
- Current Minnesota OT license in good standing
- Minimum 2 years healthcare experience (home care preferred)

- Valid driver’s license and proof of insurance for company travel
