Job Summary
Deliver skilled, patient-centered care to home health patients in the Naperville area by performing comprehensive evaluations of activities of daily living, upper extremity function, and home safety to develop individualized treatment plans. Educate patients and caregivers on adaptive techniques, assistive equipment, and energy conservation strategies while collaborating with an interdisciplinary team. Think critically to solve real-time problems and ensure thorough, compliant EMR documentation. Work independently within your assigned territory, traveling to patient locations using a company vehicle for approximately 20% of your day. This role offers a $10,000 first-year success bonus and includes a fleet vehicle program, generous PTO, and 401K matching.
Required Qualifications
- Active Occupational Therapist, OT license in the state you work
- One year of experience as an Occupational Therapist
- Clinical expertise to deliver quality patient care
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Must be able to lift 50 lbs
