Job Summary
As a Home Health RN, provide skilled nursing care to patients in their homes under physician direction. Perform start-of-care, routine, and discharge visits; develop and implement nursing plans of care; monitor patient conditions and communicate updates to the care team. Supervise LPNs, CNAs, and HHAs, and participate in interdisciplinary conferences. Travel to patient homes across Duval, St. Johns, Clay, and Nassau counties (in-person role). Maintain thorough clinical documentation, ensure HIPAA compliance, and participate in on-call rotations and training. Requires Florida RN license, valid driver’s license, CPR certification, and at least a nursing degree (Bachelor’s preferred; Associate acceptable with experience). Bilingual Spanish is a plus. The organization offers a $7500 sign-on bonus and a full-time, 40-hour workweek with salaried status and mileage reimbursement.”,
Required Qualifications
- Current Florida Registered Nurse (RN) license
- Bachelor’s degree in Nursing preferred; Associate degree acceptable with appropriate experience
- Valid Florida driver’s license with acceptable driving record, reliable transportation and current auto insurance
- Current CPR certification
- Ability to work in patient homes and various settings
- Bilingual Spanish skills are a plus
- AHCA background screening clearance (Level II) per organization requirements
- Experience: clinical experience appropriate to licensure; home health experience preferred
- Ability to document care services and maintain HIPAA compliance
