Home Health Nurse (RN or LPN)

Home Health Nurse (RN or LPN) for Giving Home Health Care performing in-home patient care under physician plans of care. 1099 contract opportunity with care delivered at patients’ residences across Arizona, Colorado, Kentucky, Missouri, Nevada, New Mexico, Tennessee, Texas, and Utah. Responsibilities include executing physician prescribed plans of care, documenting care in the system, administering prescribed medications and therapies, conducting patient assessments and care coordination, assisting with activities of daily living, monitoring vitals and intake/output, wound care, communicating with family members, physicians, case managers, and other prescribers, and providing education to patients and families. Collaborates with CNAs and HHAs; supervises care assistants; requires travel to patients’ homes and building lasting patient relationships. Qualifications include nursing licensure (RN or LPN), CPR, required screenings, background considerations, and dependable transportation; must be comfortable working in a home-health, in-home setting with potential 12-hour or 6-hour shifts.
