Job Summary
Coordinate services and care for assigned clients, including therapy. Observe, record, and report signs, symptoms, reactions to treatments, medications, and changes in physical or emotional conditions to the physician. Teach clients and family members about proper care and effective management of medical conditions. Arrange counseling services as appropriate. Complete OASIS documentation accurately and prepare discharge summaries and instructions. Schedule is 30-40 hours per week during the day. Eligible for health, dental, vision, vacation, and 403(b) matching. Hourly wage ranges from $25.75 to $42.50 depending on RN or LPN licensure. Required Qualifications
- Valid State of Wisconsin Registered Nurse or Licensed Practical Nurse license
- Current CPR certification
- Valid State of Wisconsin driver license
- Reliable vehicle and appropriate insurance
- Acceptable motor vehicle record

- Previous experience managing chronic conditions a plus
