Home Health Nurse - Casual Call

Coordinate services and care for assigned clients, including therapy, while observing, recording, and reporting signs, symptoms, reactions to treatments, and changes in physical or emotional condition. Teach clients and family members about proper care and effective management of medical conditions, and arrange counseling services as appropriate. Ensure accurate completion of OASIS documentation and handle discharge planning with preparation of summaries and instructions. Work on a casual call basis during daytime weekday hours. Requires a valid Wisconsin RN or LPN license, current CPR certification, and a valid Wisconsin driver's license with a reliable vehicle and appropriate insurance.
