Job Summary

Coordinate, manage, and deliver individualized care to patients in their homes while developing and implementing comprehensive nursing assessments and care plans. Provide skilled nursing care, treatments, and disease management education, then monitor patient progress to adjust plans and promote positive outcomes. Educate patients and caregivers regarding diagnoses, medications, and self-management strategies while ensuring compliance with Medicare, state, and organizational documentation standards. Maintain accurate electronic documentation and communicate changes in patient condition to the interdisciplinary care team. Participate in care conferences, quality improvement activities, and collaboration with physicians, therapists, and social workers.

Required Qualifications

  • Current Registered Nurse (RN) license in Iowa, in good standing
  • Graduate of an accredited nursing program
  • Valid driver's license, reliable transportation, and proof of automobile insurance that meets VNA requirements
  • Ability to travel throughout the service area
  • Strong clinical assessment, communication, organization, and critical thinking skills
  • Excellent communication, organization, and time management abilities
  • Ability to work independently while collaborating effectively with physicians, patients, families, and interdisciplinary team members
  • Ability to work independently while collaborating effectively with an interdisciplinary team
  • Successful completion of a criminal background check conducted by the Iowa Division of Criminal Investigation (DCI)
  • Verification of eligibility to work in healthcare programs, including a negative finding on the Office of Inspector General (OIG) Excluded Provider List

Desired Qualifications

  • One year of clinical nursing experience
  • Home health, hospice, case management, community health, or public health nursing experience
  • Case management experience
  • Knowledge of Medicare home health regulations and documentation requirements
  • Experience utilizing electronic medical records
  • OASIS assessment experience
