Job Summary
Assess patients' social and emotional states, financial resources, and community needs to develop personalized care plans. Provide direction to Social Worker Assistants and evaluate their performance against compliance standards. Communicate with patients, families, and interdisciplinary team members to coordinate care and resolve recovery barriers. Maintain required documentation including case conferences, visit reports, and progress notes while obtaining necessary community resources. Requires one year of healthcare social work experience, state licensure, valid driver's license, and CPR certification. Schedule is flexible with local travel; pay ranges from $65,000 to $88,600 annually.
Required Qualifications
- Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.
- Social Worker licensure in the state of practice, if required by state law or regulation.
- A valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification
- Experience with discharge planning needs, and obtaining community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility).
- Knowledge of medications and their correct administration.
- Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
- Flexibility in work hours and travel locally.
- Maintain current licensure certifications and meet mandatory continuing education requirements.
- Must read, write and speak fluent English.
- Required to be screened for TB
