Job Summary
Assess patients' social and emotional states and medical/nursing needs in the home setting to develop personalized plans of care. Provide direction and supervision of Social Worker Assistants while obtaining community resources to resolve barriers to recovery. Communicate with patients, families, and interdisciplinary team members for care coordination and case conferences. Maintain documentation including visit reports, progress notes, and confers with healthcare disciplines to ensure compliance with company and government standards. Evaluate Social Work Assistant performance and submit required reports. Requires one year of healthcare social work experience, state licensure, valid driver's license, and CPR certification. Full-time employment in Charlotte, Lee, and Collier counties with a base pay range of $53,700-$72,600.
Required Qualifications
- Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.
- Social Worker licensure in the state of practice, if required by state law or regulation.
- A valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification
- Experience with discharge planning needs, and obtaining community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility).
- Knowledge of medications and their correct administration.
- Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
- Flexibility in work hours and travel locally.
- Maintain current licensure certifications and meet mandatory continuing education requirements.
- Must read, write and speak fluent English.
- Required to be screened for TB
