Job Summary
Assess patients' social and emotional states and medical/nursing needs in the home setting to develop personalized care plans. Provide direction and supervision of Social Worker Assistants while evaluating their performance against company and professional standards. Obtain community resources to resolve issues impeding recovery and communicate with patients, families, and interdisciplinary team members. Maintain documentation including case conferences, visit reports, and progress notes in compliance with healthcare regulations. Requires a valid driver's license, CPR certification, and flexibility for travel; pay range $65,000 - $88,600 annually.
Required Qualifications
- Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.
- Social Worker licensure in the state of practice, if required by state law or regulation.
- A valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification
- Experience with discharge planning needs, and obtaining community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility).
- Knowledge of medications and their correct administration.
- Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
- Flexibility in work hours and travel locally.
- Maintain current licensure certifications and meet mandatory continuing education requirements.
- Must read, write and speak fluent English.
