Job Summary
Assess patients' social and emotional states regarding illness, injury, and treatment responses. Evaluate relationships of medical and nursing needs in the home setting, financial resources, and available community resources. Provide direction and supervision of Social Worker Assistants to ensure compliance with company, government, and professional standards. Obtain community resources to resolve issues impeding patient recovery and participate in interdisciplinary care coordination. Maintain documentation including case conferences, visit reports, and progress notes while communicating with patients, families, and healthcare disciplines.
Required Qualifications
- Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.
- Social Worker licensure in the state of practice, if required by state law or regulation.
- A valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification
- Experience with discharge planning needs, and obtaining community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility).
- Knowledge of medications and their correct administration.
- Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
- Flexibility in work hours and travel locally.
- Maintain current licensure certifications and meet mandatory continuing education requirements.
- Must read, write and speak fluent English.
- TB screening (required for this patient-facing role)
