Job Summary
Assess patients' social and emotional states, medical needs, and financial resources to develop personalized plans of care. Provide direction to Social Work Assistants, obtain community resources to resolve recovery barriers, and communicate with patients, families, and interdisciplinary team members. Evaluate Social Work Assistant performance and maintain documentation including case conferences, visit reports, and progress notes. Requires a valid driver's license, current CPR certification, and one year of home health social work experience. Full-time employment with a salary range of $53,700 to $72,600.
Required Qualifications
- Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.
- Social Worker licensure in the state of practice, if required by state law or regulation.
- A valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification
- Experience with discharge planning needs, and obtaining community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility).
- Knowledge of medications and their correct administration.
- Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
- Flexibility in work hours and travel locally.
- Maintain current licensure certifications and meet mandatory continuing education requirements.
- Must read, write and speak fluent English.
- TB screening (required for patient-facing role)
