Job Summary
Medical Social Worker for Home Health PRN role providing patient-centered social work within interdisciplinary teams. Responsibilities include assessing patients' social/emotional needs related to illness or injury, evaluating relationships with medical/nursing needs in the home, coordinating community resources, participating in care coordination with the care team, supervising the Social Worker Assistant when involved, and maintaining documentation of visits, conferences, and communications. Requires at least one year of relevant healthcare social work experience, a valid driver's license and transportation, ability to perform discharge planning and resource acquisition, and fluent English. Based in Kalamazoo County, MI. Wear alignment with CenterWell Home Health and Humana policies.
Required Qualifications
- Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice
- A valid driver's license, auto insurance, and reliable transportation
- Experience with discharge planning needs, and obtaining community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility)
- Flexibility in work hours and travel locally
- Must read, write and speak fluent English
