Job Summary
Assess patients' social and emotional states regarding illness, injury, and treatment adjustments. Evaluate relationships between medical needs, financial resources, and community assets to resolve barriers impeding recovery. Provide direction and supervision of Social Work Assistants while participating in interdisciplinary care coordination. Maintain documentation for case conferences, visit reports, and progress notes in compliance with company and governmental standards. Communicate with patients, families, and healthcare disciplines to ensure optimum care.
Required Qualifications
- Masters license
- Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice
- Social Worker licensure in the state of practice, if required by state law or regulation
- A valid driver's license
- Auto insurance
- Reliable transportation
- Proof of current CPR certification
- Experience with discharge planning needs
- Experience with obtaining community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility)
- Knowledge of medications and their correct administration
- Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations
- Flexibility in work hours and travel locally
- Maintain current licensure certifications and meet mandatory continuing education requirements
- Must read, write and speak fluent English
- Must be screened for TB
