Job Summary
Assess patients' social and emotional states, medical needs, and financial resources to develop personalized care plans. Obtain community resources to resolve barriers to recovery and communicate with patients, families, and the interdisciplinary care team. Evaluate Social Work Assistant performance and maintain documentation including case conferences, visit reports, and progress notes. Participate in care coordination activities while adhering to company policies, healthcare standards, and governmental regulations. Maintain current licensure, CPR certification, and required continuing education.
Required Qualifications
- Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.
- Social Worker licensure in the state of practice, if required by state law or regulation.
- A valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification
- Experience with discharge planning needs, and obtaining community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility).
- Knowledge of medications and their correct administration.
- Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
- Flexibility in work hours and travel locally.
- Maintain current licensure certifications and meet mandatory continuing education requirements.
- Must read, write and speak fluent English.
- Must have a Masters license
