Job Summary
Home Health Medical Social Worker in Jackson, MI. Provide psychosocial support to patients and families to help follow treatment plans, access community resources, and work toward independence. Collaborate with nurses, therapists, and physicians to ensure emotional support, discharge planning, and care coordination. Responsibilities include completing initial social work evaluations within 5 days of start of care, assessing social/emotional factors affecting adherence, counseling for adherence and well-being, connecting patients with resources, developing individualized care plans, participating in multidisciplinary conferences, and maintaining timely documentation. Requires an MSW/LMSW/LCSW and State license, travel daily within the service area, and ability to work independently while coordinating with the care team.
Required Qualifications
- Master's or doctoral degree from a CSWE-accredited school of social work
- Current State License as a Master's Social Worker (MSW/LMSW/LCSW)
- Minimum 1 year of experience as a licensed Master's Social Worker
- Reliable transportation with valid automobile insurance
- Willingness to travel daily for home visits within service area
