Job Summary
Assess psychosocial needs, connect patients with community resources, support care planning, and advocate for goals that promote dignity, stability, and independence in the comfort of patients' homes. Provide compassionate, patient-centered support for people and families navigating the emotional, social, and practical challenges of illness and recovery. Maintain meaningful 1:1 patient relationships with autonomy and flexibility, utilizing the Epic EHR system for efficient documentation and seamless care coordination. This role offers consistent, reliable workloads with predictable schedules and no on-call requirements, allowing for true work-life balance.
Required Qualifications
- Master's Degree from a school of social work accredited by the Council on Social Work Education
- State of Maryland Social Work License
- Two (2) years of social work experience in health care setting
- Ability to provide service to acutely and chronically ill patients
- Ability to complete written, verbal, and observational tasks indigenous to the position
