Provide social work services in accordance with the physician's plan of treatment, assessing and reassessing social and emotional factors to help patients and families cope with daily living problems. Counsel patients and families regarding the facilitation of the medical plan of treatment while recognizing and utilizing community resources for long-term life plans. Develop care plans with case managers, physicians, and families to address personal, financial, and environmental difficulties, setting appropriate goals and participating in discharge planning. Prepare and maintain individualized patient care progress records with accuracy and timeliness. Requires an active Michigan Medical Social Worker license, reliable transportation, and valid driver's license and auto insurance.
