Home Health Medical Social Worker

Conduct home visits to clients in Waltham, Newton, Weston, Wayland, and surrounding towns to provide clinical social evaluations and plan interventions. Identify community resource referrals to address practical and environmental needs, acting as an advocate to navigate the patient through the community system. Participate in developing the total plan of care with the multidisciplinary team and educate patients and families on available resources. Maintain contracts with public and private agencies and accurately document observations and evaluations using a touch pad tablet.
