Job Summary
Home Health LVN responsibilities include traveling to patients in SNF, assisted living, and home settings under the direction of the RN, identifying patients' physical, psycho-social, and environmental needs, providing services ordered by physicians, participating in training, and managing a caseload of 40 to 50 patients per week. Requires active LVN license in Texas, Oasis experience, and familiarity with EMR systems. Coverage area includes Allen, Plano, Wylie, Garland, McKinney, Richardson, Rowlett, and Sachse.
Required Qualifications
- Registered in the State of Texas
- 1 Year of Home Health Experience-Preferred
- Oasis
- Familiarity with EMR systems -Preferred
