Job Summary
PRN Home Health LVN position in San Marcos, TX, providing in-home nursing care under RN supervision. Responsibilities include identifying patient physical, psychosocial, and environmental needs; planning and implementing care in partnership with patient, caregiver, and RN; monitoring cases for third-party payor requirements; participating in QAPI and agency trainings; delivering patient and caregiver education; adhering to infection control measures; ensuring continuity of care through timely documentation; and coordinating with healthcare professionals. Requires graduation from an accredited school of vocational/practical nursing, active LVN/LPN license, and one year of clinical LVN experience; Homecare Homebase experience is a plus.
Required Qualifications
- Education: Graduate of an accredited school of vocational/practical nursing
- Licensure: Active, state license as a licensed vocational/practical nurse (LVN/LPN) or multi-state license
- Experience: One year of experience as a licensed vocational/practical nurse in a clinical care setting required
- Home health experience preferred
- CPR certification
