Home Health LVN

Assist in identifying patient physical, psychosocial, and environmental needs through documentation. Participate in planning and implementing care in partnership with the RN, patient, and caregivers while providing physician-ordered services. Deliver patient and caregiver education, maintain infection control measures, and ensure continuity of care. Provide effective communication to patients, families, and team members via clinical notes and case conferences. Monitor assigned cases for third-party payor compliance and participate in QAPI programs and in-service trainings. Requires one year of clinical LVN experience, active license, CPR certification, and reliable transportation. Payrate is $35-40 per visit.
