Home Health LVN

Halcyon Home is hiring a Home Health LVN Full Time to support geriatric home health in San Marcos, TX and surrounding areas. Under the direction of the RN, the LVN assists in identifying patients' physical, psychosocial, and environmental needs; provides effective communication among patients, families, team members, and healthcare professionals; completes documentation; monitors cases for third-party payor requirements; participates in QAPI and in-service trainings; demonstrates competency and professional growth; promotes organizational philosophy and care quality; and plans/implements care in partnership with patients and caregivers, delivering care per the physician's plan of care with patient and caregiver education, infection control, and continuity of care. Will be present within partnering communities and clients' residences, with responsibilities including care planning, education, and collaboration with RNs to ensure quality patient care.
