Job Summary
Assist in identifying patients' physical, psychosocial, and environmental needs through documentation while participating in care planning with the RN, patient, and caregivers. Monitor assigned cases for third-party payor compliance, deliver physician-ordered services, and provide patient and caregiver education using infection control measures. Ensure continuity of care by completing clinical notes, attending QAPI programs, and in-service trainings while promoting agency policies. Requires an active LVN license, one year of clinical experience, CPR certification, and reliable transportation. Payrate is $35 per visit.
Required Qualifications
- Graduate of an accredited school of vocational/practical nursing
- Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure
- One year of experience as a licensed vocational/practical nurse in a clinical care setting
- Proof of current CPR certification
- Reliable transportation
- Valid auto liability insurance
