Job Summary
Deliver in-home nursing care as a Home Health LVN/LPN under RN supervision, including patient health assessment, medication administration, and wound care. Develop and implement individualized care plans, educate patients and caregivers on treatment protocols, coordinate with physicians, therapists, and family, and ensure infection control and quality of care. Monitor cases for third-party payor requirements and support chronic disease management and recovery in the home setting.
