Job Summary

Provide LPN/LVN-level hospice nursing services under RN supervision, implementing interventions per the Plan of Care and reporting patient data to anticipate disease progression. Educate patients and families on care needs, insurance benefits, and cost-effective management while documenting observations and communications accurately. Manage assigned caseloads, collaborate with the hospice team through effective communication, and perform triage and medication desk duties as assigned. Maintain awareness of professional scope, compliance with hospice Conditions of Participation, and quality improvement activities. Requires California LVN licensure, infusion certification, and BLS; offers base pay of $31.11-$47.59 with shift differentials and comprehensive benefits.

Required Qualifications

  • California Licensed Vocational Nurse required upon hire
  • Infusion and Venipuncture Certification required
  • National Provider BLS – American Heart Association within 30 days of hire
  • Valid driver's license and auto insurance required upon request
  • Minimum 1 year as an LVN
  • 6 months home care preferred
