Job Summary
Home Health LVN providing hands-on care to patients in Hesperia and surrounding areas (Hesperia, Oak Hills, Phelan, Pinion Hills, Victorville, Adelanto). Role involves medication management, wound care, health monitoring, and using AI-supported tools to complete clear, accurate visit notes. Works Monday to Friday with one weekend rotation per month; must be local and willing to travel to listed cities. Emphasis on patient-centered care and reducing documentation burden to allow more time for direct patient interaction. Requires CA LVN license, infusion and venipuncture certification, BLS within 30 days of hire, and valid driver’s license.
Required Qualifications
- Graduate of an accredited Licensed Practical/Vocational Nurse program
- California Licensed Vocational Nurse license upon hire
- Infusion and Venipuncture Certification
- National Provider BLS – American Heart Association within 30 days of hire
- Valid driver’s license and auto insurance upon request
