Home Health LPN

Conduct patient assessments, administer medications, manage wounds, and monitor vital signs in home settings. Document all care activities in EMR systems and collaborate with RNs, therapists, and physicians to execute established care plans. Educate patients and families on treatments and disease management protocols.
