Job Summary
Provide skilled nursing interventions for patient illness, rehabilitative needs, and preventative care under RN direction. Confer with the RN Supervisor regarding treatment plan changes and maintain contact with patients, physicians, and the clinical management team regarding status updates. Participate in care coordination activities and discharge planning while preparing medical documentation, visit reports, and shift summaries. Maintain a valid driver's license, auto insurance, and reliable transportation for patient visits.
Required Qualifications
- Licensed Practical Nurse, licensed in the state of North Carolina, without restrictions
- A valid driver's license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Must be able to lift 50 lbs
- Must be available for weekend shifts
- Valid driver's license
- Proof of personal vehicle liability insurance with at least 100/300/100 limits
- Undergo TB screening
