Job Summary
Deliver skilled, patient-centered care to home-based patients in the Moscow, PA area, performing comprehensive assessments including vital signs, medication management, and wound care. Educate patients and caregivers on disease management and prescribed treatments while maintaining accurate EMR documentation. Work independently within an assigned territory, traveling to patient locations approximately 20% of your day. Requires an active LPN license, two years of clinical experience, and a current driver's license. Eligible for a $10,000 first-year success bonus and a pathway to an ASN degree with up to $20,000 in student loan repayment.
Required Qualifications
- Active Licensed Practical Nurse (LPN) license in the state you work
- Two years of LPN experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Must be legally authorized to work in the United States
- Successful completion of a drug screen
